Employed Providers & Staff Login

Employed staff should click "Sign-in with @endeavorhealth.org Email & Password" to sign in to their CloudCME™ account. This will be the same username and password you use for other Endeavor Health applications and utilizes a single sign-on process.

Please note that you are only able to utilize single sign-on offsite if your device is set up to remotely connect to the network.

Non-Employed Providers Login

All other users should click "Sign-in with Personal Email & Password" to sign in to their CloudCME™ account.

New Activity Proposal

To submit an application for a new activity proposal, please email [email protected] prior to beginning the application process so we can ensure you understand the process and have appropriate access and review our Resources page.

Please be sure you have discussed your activity with the CME department and review the planning guidelines before applying for credit. In order to submit an application for a new activity, please complete the documents on the left-hand side of this page. Please fill out the application with as much information as possible and contact the department with any questions. Once completed, email to [email protected] to get started.

Learner Completion Data

Please note that we submit learner completion data for all U.S. physicians to the ACCME Program and Activity Reporting System (PARS). AMA PRA Category 1 Credit(s)™ data, and Maintenance of Certification (MOC) credits (where offered) submitted to PARS can be viewed, managed, and tracked by U.S. physicians on CME Passport.

If you obtain AMA PRA Category 1 Credit(s)™ or Maintenance of Certification (MOC) credits (where offered) from other institutions, and they submit this data, you will be able to see all of your credits in a centralized location. If you do not wish for your credits to be transmitted, you must OPT OUT within your CloudCME account profile (MyCME > Profile).
