Overview
Diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS) are the two most common hyperglycemic emergencies inpatients with type 1 and type 2 diabetes. DKA accounts for 4-9% of all hospital discharge summaries among patients with diabetes, which, in the United States, is an annual average of greater than 170,000 hospitalizations for DKA. The mortality rate is less than 2%, but the prognosis of both conditions is substantially worsened in patients >65 years of age and in the presence of coma and hypotension. Precipitating causes of DKA/HHS are primarily due to stopped insulin, new onset diabetes, pancreatitis, ETOH (alcohol) abuse and infections. The emergence of COVID-19 in December 2019 has been a factor in increased cases of DKA, complicating management due to increased steroid use and acute renal insufficiency. Likewise, admissions for euglycemic DKA have increased with use of SGLT-2 inhibitors, requiring additional considerations in optimal medical management. DKA/HHS medical management in the ICU requires a team approach with detailed knowledge of fluid, electrolyte, and insulin restoration to recover the patient with DKA/HHS. This activity has been designed to review the pathophysiology of DKA, address key factors of management by examining best practice therapeutic approaches and case study review.

Objectives

At the conclusion of this activity, participants should be able to:


  1. 1) Apply key therapeutic approaches to diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS) management in the ICU.
  2. 2) Explain the maintenance phase of diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS) management in the ICU.
  3. 3) List the transition phases of diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemic state (HHS) management in the ICU.
