Overview
Through evidence-based review of formularies the Pharmacy and Therapeutics Committee analyzes medication-use, adverse drug event monitoring and reporting, medication error prevention, medication safety, development of clinical care plans, and medication management initiatives such as delegation and practice protocols, restrictions, guidelines, and clinical pathways. The formulary system is the multidisciplinary and interdisciplinary evidence-based process employed by Edward-Elmhurst Health to select and use medications that offer the best therapeutic outcomes while minimizing potential risks and costs for patients. This conference evaluates the hospital's formulary, updating the list of medications and related information. Evidence-based guidelines evolve as new treatments options and research is brought forward; hospitals and practitioners must be cognoscente of current recommendations and use this knowledge to update existing approaches or develop new approaches to medication use. It is also important for providers and staff to be educated through unbiased reviews of the medical literature through the process of both the formulary decision-making and addressing the overall medication use throughout the system through the representation of clinical judgment from physicians, pharmacists, and other experts in the diagnosis, prevention, or treatment of disease and promotion of health.

Objectives

At the conclusion of this activity, participants should be able to:

  1. 1. Determine the role of the identified medication in clinical practice.
  2. 2. Explain the impact guidelines, orders sets and medication evaluations have in the inpatient setting.
  3. 3. Assess the safety data and recommend measures for safe medication usage and make appropriate recommendations.
